Please provide 48-hour advance notice for appointment cancellations. Missed appointments and late cancellations will be charged the cost of the appointment. Please be aware that insurance does not cover these fees.
First-time cancellations within 48 hours of the scheduled appointment will not be penalized. Your health care needs are important to me. If there are any challenges that you are facing that make it difficult to meet with me, please let me know and we can try to work through them together.
Second-time cancellations within 48 hours of the scheduled appointment will result in a fee equivalent to half the amount of the normal visit rate.
Third-time cancellations within 48 hours of the scheduled appointment will result in a fee equivalent to the the full amount of the normal visit rate. Third-time cancellations may result in dismissal from the clinic.
If you arrive 15 minutes late to your appointment, I may ask that we reschedule to an alternative date/time.
When requesting medication refills outside of scheduled appointments, please notify me at least 3 business days in advance, ensuring that you do not miss doses of medications, and allowing me time to send refill prescriptions to your pharmacy. I do not process refill requests on weekends or holidays.
While some patients may benefit from medications such as stimulants (Adderall, Ritalin, Concerta, etc.) and benzodiazepines (Xanax, Ativan, Klonopin, Valium, etc.), I only prescribe these medications in select situations for well-established patients for specified periods of time. These medications are highly regulated by the federal government due to risks of harm, dependence, and abuse. When using these medications, we will regularly review the risks/benefits, fulfill laws requiring face to face encounters for ongoing prescriptions, and create treatment plans focused on tapering these medications to the lowest effective dose for shortest duration, and discontinuation.

If you are a family member or a friend who has safety concerns, please call the non-emergent police line (503) 823-3333 and request a welfare safety check (prepare name, address, contact information). For acute safety concerns, please call 911 (medical) or 988 (mental health).
At the conclusion of care, you will be provided a 30-day supply of medications, unless otherwise arranged. You will be mailed a letter terminating our patient-provider relationship. If you are transitioning care to a new provider, please complete a Release of Information form and return to me via fax (503-300-4691), which will allow me to share relevant records to support your transition of care.
